We don’t just look at medical records; we look at the person behind them. We want to know what makes you smile, what your daily routine looks like, and what your big goals are for the year ahead.
By bringing together your family, your doctors, and our expert team, we create a “living” roadmap. This plan is designed to be a flexible guide that grows and changes right alongside you, ensuring that every hour of care we provide is purposeful, respectful, and exactly what you need.

Planning Your Care Together
Starting a new care journey can feel like a big step. We want you to feel completely comfortable with how we build your support plan. Here are the answers to the questions families ask us most often about the assessment process.

How long does the assessment take?
We usually spend about 90 minutes to two hours with you. We don’t want to rush; we want to make sure you have plenty of time to tell us your story and ask any questions you might have.

Who should be there for the assessment meeting?
It’s completely up to you! We recommend having a trusted family member, a friend, or an advocate there to support you, but the most important voice in the room is always yours.

What if I don't agree with something in the care plan?
The plan isn’t finished until you’re happy with it. We work in partnership with you. If there’s something you’d like to change or a goal you’d like to add, we simply update the plan together.

How often do we update the plan once it’s finished?
We do a formal review every six months, but we’re always listening. If your health changes or you achieve a big goal sooner than expected, we can update the plan at any time.
